How to register
Please note that to register with the practice you need Proof of address. This is to make sure that you are in our catchment area (live within the surgery area)
- Current Utility Bill (no older than 3 months)
- Bank Statement (no older than 3 months)
- Tenancy Agreement
- Any other official letters that prove you are at the address you are registering at.
Identity
- Current Passport
- European Identity Card
- New Full Driving License
- Children – original birth certificate
- Children – Red book
NHS Medical Card
Immunisations – adults and children – please provide proof
Once you are registered with the surgery, an appointment can be booked to see a Doctor.
Live within boundary area?
To be eligible to register at the practice, you must live within the practice catchment area.
